In my school they have a disabilities services center where you can also register with and try to get needed accommodations at the school. It is up to the student to seek out this service.
Although most faculty will be willing to listen if you are trying to get help, trying to advocate for yourself can be much more successful than relying on school disability resources. This just requires you to know (or try to explain) what your needs are and learn how to convey them if possible to your teacher or classmates so they can help you relate to the material.
If you are a first college student at your school they may have something like a TRIO program where they can give you a little extra help navigating college that you wouldn't get if your family did not have those experiences to share. You also get the additional benefits they have of different tutors and resources like books on top of the college's resources.
I have also utilized tutoring available through my college to help with math and writing.
Succeeding never makes you feel bad about the help you had to get to get there. For me it helped me realize there is hope and good folks out there in the world that can help.

IFSP
Individualized Family Support Plan under IDEA (Individuals with Disabilities Education Act) is for birth to three years old that have delays in specified areas. -
IEP
Individualized Education Plan under IDEA is available to qualifying children ages three to twenty-one years old and can include a modified core curriculum and remediation. -
ADA and Section 504
Although the IEP does not follow through to college, if it can be updated to help the disability departments in the higher education facility with accommodations within their abilities.
